Medical Network One is taking the lead in introducing Michigan to a growing health care movement called the patient-centered medical home, (also referred to as a patient-centered advanced medical home) a patient-centered, physician-guided model of health care.
The patient-centered medical home profoundly changes the way primary care is delivered by connecting patients to personal physicians who follow advanced medical home philosophies. It requires in-depth physician and staff training and changes in third party financing, reimbursement methods, and policy coverage to support practices that meet advanced medical home guidelines.
The patient-centered medical home is rooted in the Chronic Care Model (CRM) of medicine, a model that Medical Network One has championed since 2002 in dealing with such chronic illnesses as diabetes, asthma and heart disease. Medical Network One’s Chronic Care Travel Team program not only offers chronic disease management services in a cost-effective manner but lays the foundation for the creation of a patient-centered advanced medical home. The advanced medical home presents a major thrust for changing the way physicians practice medicine; it’s a shift from an I/physician-centered practice to a team/patient-centered practice.
